These forms may be downloaded, printed, completed  and mailed to The Registry.    The first two are in .pdf format and require Adobe® Reader™ to use.  This may be obtained on-line directly from the Adobe website without charge (www.adobe.com). 

The Database Information Form may be used to change your directory listing.  It is  in MS Word form-fill-in format. Complete the form, save it, and return the file as an e-mail attachment to admin@california-registry.org.  Alternatively, it may be printed and mailed to The Registry.
